At least there was not such a system until this week. This week www.theoriginalmymedicalrecords.com announced the prototype release of Version 1.2, a portable medical record with change tracking, high-level encryption and password protection.

None of these systems however have one basic piece that is required for credibility in the medical world. It is called change tracking and it is the ability for the healthcare professional reading the portable medical record to look at what changes have been made in the portable medical record and what existed in the record before the change was made. Change tracking is an internal audit of the system and ensures that there has been no tampering that could threaten a patient's life.

It is amazing how once you notice something, you begin to pay attention and in paying attention you discover a whole new world.

For those of you who are not familiar with portable medical records allow me a moment to give some definitions. A portable medical record is a USB drive device such as a Thumb Drive, a USB wristband, a USB flash drive wristwatch, pendant, or other portable mass storage that can be worn on the body attached to a belt or keys and holds basic medical information in a database form. Almost all these devices now are password protected and offer varying levels of functionality.

But I envision a larger market. I envision a day when rather than receiving a flimsy paper wristband at the hospital or nursing home a patient has a portable medical record in a waterproof band around their wrist. When the nurse comes by with their wireless Tablet computer to chart she simply plugs the patient record band in and the records are immediately synchronized. The patient goes nowhere without their chart on their wrist.

Yes, they all have some form of password protection and an emergency screen where basic information can be seen without the use of the password. They hold information such as living wills, organ donor cards, healthcare surrogate contracts, past medical histories, allergies, medications and a few hold greater levels of data. Two of them integrate with online personal health records but only one can import and export to electronic medical records.

With that in mind let's turn now back to the portable medical records. This is a market that is exploding. A few short weeks ago I wrote an article, my second in this arena, that described nine products of this type. Two days ago I did a new Google search on this topic (okay, I was desperate for an article topic) and I found no fewer than 24 companies now offering these devices in the United States, Canada and England. Many of these devices are now being made in Taiwan and China and installed directly on the flash drives. They are coming in every shape, size and form but unfortunately with little or no functionality.

As the water resistant or splash resistant USB devices these units are already finding a home on the wrists of scuba divers, sky divers, mountain bikers and others who enjoy high fun/ high risk sports.

A personal health record is the online equivalent of the portable medical record. This software exists on the Internet with storage maintained at a third-party site. The information is again password protected and in an emergency can be accessed by the healthcare provider with an Internet connection and that password.

I envision a day when my disaster medical assistance team (MDMS/DMAT-FL3) provides care in a Katrina like event after a natural disaster with subsequent flooding and places a USB flash drive wristband around the wrist of every evacuee. Information on federal assistance, registration for finding lost family members, their own personal information as well as a health record will travel with them from the moment of rescue until their final destination. If they already had a personal medical record on their wrist or in their pocket or around their neck as an independent we in the MDMS/DMAT-FL3 would be able to plug their device into our computers and upload the important information to help them recover their lives and help us treat their injuries.

The human immune defense system consists of two main components, the innate and adaptive. The innate component is so named because it is a general defense system. Some of the tissue based immune cells destroy the invaders by actually eating them (phagocytes, eating cells and macrophages, large eating cells). The innate immune response is the automatic defense mechanism against potential foreign invaders such as bacteria, viruses, molds or parasites. It is analogous to defensive barriers, alarms and guards intended to protect or defend us personally or as a society. Such defensive barriers around our borders or sites of value to us may include protective walls, electric fences, locked doors, security guards and automatic alarm systems. Such systems may include automatic motion activated lighting and alarm systems, and low level security guards. This defense though highly effective for most threats, is not very specific, highly trained or lethal against serious threats. It also can be penetrated or breached easily though it may be linked to an alarm system that can signal for help. Therefore, we need the protection of more highly trained and specific defenses of the adaptive immune response.

THE TWO COMPONENTS OF THE HUMAN IMMUNE DEFENSE SYSTEM:

The intestinal lining is normally an intact barrier except when signaled to allow some bigger particles through such as certain proteins or other nutrients. The intestinal tract lining has a single layer of cells of the epithelial cell type. These cells are joined tightly, shoulder to shoulder, through a scaffolding of proteins, such as occludens. These tight junctions can open up on signal, from proteins such as zonulin. Certain normal signals permit the opening of the gaps between the lining cells, the paracellular spaces, or decrease the tight junctions resulting in increase intestinal permeability transiently. However, abnormal stimuli can trigger opening of the tight junctions resulting in abnormally increased intestinal permeability or leaky gut. When tight junctions open up wall of the gut may become abnormally permeable or leaky allowing it to be penetrated by foreign proteins including bacteria, viruses, molds, parasites and intact food proteins or lectins. Impairment of gut wall integrity or barrier function is an abnormal innate immune defense. It is like an insecure border, fence or wall.

The adaptive immune system also includes specialized activated lymphocytes: natural killer T cells, T helper lymphocyte cells, and gamma delta lymphocytes. The latter are also part of innate immune system. Tiny brief lived cell called the plasma cell are also part of the adaptive system. Because the adaptive system has a memory, and is highly trained for future anticipated attacks it can increase its response over time. That makes the adaptive system highly effective in defending us against foreign invaders such as serious infections but also highly dangerous to us if it becomes activated against our own cells or tissues.

The adaptive response includes the development of specific antibodies or immunoglobulin (IgE, IgM, IgG, IgD, IgA) to infectious agents and proteins the body recognizes as foreign (antigens). Sometimes the body misinterprets or mistakes self-proteins or tissues as foreign. When antibodies are made against self-proteins they are called autoantibodies. Examples include antinuclear antibodies (ANA) of lupus and rheumatoid factors or antibodies of rheumatoid arthritis. The result is these classic autoimmune disorders.

The adaptive response is one that is highly developed and specialized (trained) in humans. It has a memory. When combined with alarm systems from the innate immune system, a rapid, specific, and directed immune response that is also quite powerful can be activated or triggered. However, when such a system is activated, it can also lead to serious collateral damage to the surrounding tissues and cells. If the response bogs down and cannot deactivate itself an ongoing autoimmune condition without an exit strategy can result. This can result in chronic injury to the cells and tissues that is characteristic of what we know as autoimmune disease.

However, with this amount of power or force available, innocent bystanders can be injured or killed if there is poor communication or identification of threats or potential invaders. This is how the adaptive immune response works.

Our innate immune defense response is then automatic and a nonspecific system. It consists of the barriers such as the skin and the intact intestinal wall of the digestive tract. The intact intestinal wall is similar to a fence or wall. It is patrolled by certain immune cells that are like security guards or rent-a-cops, guarding the body. This includes white blood cells circulating in the blood or present in the intestinal lining. Circulating white blood cells typically include cells known as neutrophils, eosinophils, and natural killer T lymphocytes. Lining the intestinal wall (skin and respiratory tract) are other white blood cells and immune cells such as basophils, dendritic cells, phagocytes and macrophages. In the intestine white blood cells present in small numbers may be mobilized to the area for specific threats. These include eosinophils, mast cells and neutrophils.

What they teach the average college undergraduate about genetics now, was not known when I went to PA school. It is imperative a practitioner keep up their medical knowledge. It can be done with very high quality information or it can be not so high quality. It could be the bare minimum number of hours or the required minimum could be just a smattering of many hours they accumulate each year. This information could be available as a one page document that is on line or in the office for patients to read. Certifying bodies like State Medical Boards and Professional associations like the American Academy of Family Physicians have a set of criteria that members of those organizations or licensees of the Board must meet on a continuing basis. There is a higher incident of haemorrhoids (piles) in Irritable Bowel Syndrome and can cause bleeding from the rectum. Bleeding from the rectum is, however, a red flag symptom and can point to serious underlying disease. Check it out.

There were no clear guidelines as to what symptoms or combination of symptoms lead to a diagnosis of IBS until the Manning Criteria was established in 1978.

Irritable Bowel Syndrome falls under the category of Medically Unexplained Disorder or Disease (MUD). Also included in this category is Fibromyalgia Syndrome. Professor Carson at the Royal Edinburgh Hospital says MUD develops when the brains control of the body becomes faulty.

External hemorrhoids treatment is something millions of middle-aged individuals have to deal with, and fortunately there are many options to choose from. However, the information and choices around these treatments can be overwhelming, and sometimes hard to understand.

Hemorrhoids, also commonly referred to as emerods or piles, is a condition that affects up to 85 percent of the elderly population at any given time. It is a condition characterized by swelling, itching and painful irritation to the veins in the anal and rectal areas. They can be both internal and external, though the majority of the people that get them, have the external variety.

Medically, the treatment for external hemorrhoids ranges from topical to very invasive. The most common method is using a topical medicated cream like Anasol or Preparation H. This causes the hemorrhoids to shrink down, and often is the only application needed. When medical attention is imperative, there is a variety of protocols that can be used.

Those with hemorrhoids can use a natural supplement called bromelain and it is a natural inflammatory found in pineapple, but is also available in pill form. This helps to reduce swelling and inflammation without the use of topical medications. In some cases, drinking 99 percent pure aloe juice has had similar effects, along with putting aloe vera on the affected area. If the hemorrhoids are particularly painful, one can use witch hazel or honey topically, generously applying it directly to the affected area.

There are many causes, and some people are more likely to get them than others. Genetic predisposition to hemorrhoids is thought to be one of the reasons that make people more susceptible to getting them. They can also be caused by straining during bowel movements, pressure on the rectal veins due to poor muscle tone and poor posture. Obesity and sedentary cause undue stress on the rectal veins, as well as does frequent constipation. In 2004 the median annual income was $69410. For new graduates it was $64536. The lowest 10% earned <$37320 and the highest >$94880. The middle 50% were between $57000-$83500.

In 2004 there were 62000 PA jobs. There were more jobs than PAs to fill them as many PAs held more than one job. Employment in physician offices accounted for 50% of all PA employment. Hospitals accounted for another 25% and outpatient care centers in HMOs, colleges and universities, prisons, professional schools and self-employment accounting for the remaining 25%.

Physician Assistants in 2008 graduated from 136 different accredited programs. After they pass the national certifying exam (NCCPA) they are able to practice. Ninety programs give masters degrees on graduation and the other 46 give either a bachelors or associates degree.

Those working in hospitals had median income in 2004 of $70310, those in physician offices and outpatient clinics $69210.

Learning the market, where the pain for employers is, and relieving that pain is a good way to earn an excellent income as a PA.

According to the Bureau of Labor -

There is an expected increase in demand of 27% for more PAs from 2004-2015. Even with current graduation rates, supply will not keep up with demand. Though PAs may incur some hefty debt, they can work in rural areas and get part or all of the debt forgiven.

Since it is painless and non-invasive it is well tolerated especially by young children. Also because the testing can be ordered without a physician and the sample obtained in their home using a kit obtained from Enterolab it is convenient. The kit is returned by overnight delivery to Enterolab who forwards the test onto Bonfils. The cost is $149 for the genetic testing alone and has to be paid for in advance by credit card or money order and is generally not reimbursed by insurance.

Understanding the Genetics of Gluten Sensitivity

Rarely, true Celiac disease or dermatitis herpetiformis, the skin disease equivalent of Celiac, have been reported to occur in people who do not have DQ2 and/or DQ8. However, according to unpublished data from Dr. Ken Fine of Enterolab, the other six types, except DQ4, are associated with risk of elevated stool antibodies to gliadin, the toxic fraction of gluten, and/or tissue transglutaminase (tTG) an enzyme. Both of these antibodies are usually elevated in the blood of individuals with Celiac disease though they may be normal in the blood of individuals who are gluten sensitive and have a normal small intestine biopsy but respond favorably to a gluten-free diet.

The severity of the sensitivity to gluten does appear to be related to the DQ type, family history (highest risk is in the non affected identical twin of a Celiac), pre-existing intestinal
injury, degree of exposure to gluten (how frequent and large a gluten load an
individual is exposed to), and immune status. Once initiated, gluten sensitivity
tends to be life long. True Celiac disease does require lifelong complete gluten avoidance to reduce the increased risk of serious complications such as severe malabsorption, cancers (especially of the GI tract) lymphoma, other autoimmune diseases and premature death due to these complications.

It is becoming obvious to many of us who have personal and professional medical experience with gluten intolerance and Celiac disease that the problem of gluten sensitivity is much greater and extends beyond the high risk Celiac genes DQ2 and DQ8. Traditionally it is reported and believed by many that if you are DQ2 and DQ8 negative you are unlikely
to have Celiac disease or ever develop it. This cannot be said with 100% certainty especially since there are documented cases of Celiac disease and the skin equivalent of CD, known as dermatitis herpetiformis (DH), in individuals who are DQ2 and DQ8 negative.

Why some people get Celiac Disease or become gluten sensitive is not well
understood. Certain factors are believed to trigger it including the onset of puberty, pregnancy,
stress, trauma or injury, surgery, viral or bacterial infections including those of the
gut, medication induced gut injury or toxicity e.g. non-steroidal anti-inflammatory
medications such as aspirin, ibuprofen, etc., immune suppression or autoimmune
diseases. There is also a well known group of individuals who are termed "latent" Celiacs. They are those at high risk because they have close relatives who have Celiac disease with whom they share one or more of the Celiac genes DQ2 and/or DQ8. Though they usually have little or no symptoms they may have abnormal blood tests and/or biopsies indicating possible or definite Celiac disease. Others have negative blood tests and normal biopsies but symptoms that respond to a gluten-free diet.

Though the lay public and many clinicians are finding the genetic tests helpful, many, including most physicians, do not understand the genetics of gluten sensitivity. We are awaiting Dr. Fine's published data on the significance of stool antibody tests and their association to the other DQ types as his lab is the only lab offering the stool antibody tests in the U.S. Other Celiac researchers in U.S. have failed to reproduce his assay but scattered reports in the literature are appearing including a recent article in the British Medical Journal indicating stool antibody testing is feasible, non-invasive, and using their protocol, highly specific but not sensitive for Celiac disease in children.

Stool antibody test results would support this and the concept of a spectrum of gluten sensitivity that is much broader and in need of better diagnostic definitions. I am an example of someone who is DQ2/DQ7 who has normal blood tests for Celiac disease but abnormal stool antibody tests and symptoms that have responded to a gluten-free diet. The strict criteria for diagnosis of Celiac disease by abnormal blood tests and a characteristic small intestine biopsy showing classic damage from gluten is much narrower than what is being seen clinically.

Again, DQ testing can be done from cells from blood or by a swab of the inside of the mouth but not all labs test for or report the full DQ typing but only the presence or absence of DQ2 and DQ8. The lab that performs DQ testing is usually determined by an individual insurance company on the basis of contracts with specific commercial labs. However, if your insurance contracts with Quest Labs or the Laboratory at Bonfils (Denver, CO) full DQ can be done if ordered and authorized by the insurance company.

All of us have patterns of proteins on the surface of our white blood cells. The proteins are known as human leukocyte antigens (HLA), one of which is DQ. Celiac Disease (CD) and non-Celiac gluten sensitivity (NCGS), and several autoimmune conditions occur more frequently with certain HLA DQ types. DQ gene testing is performed by analyzing cells from a blood sample or from a Q-tip swab of the mouth. HLA types have a naming system that can be confusing even to scientist and physicians but here is my explanation of the testing, the results, and what they may mean to you and your family.

Fine has publicly reported that elevated stool antibodies to gliadin and/or tTG have been detected in all of the untreated Celiacs tested in his lab, 60% of non-Celiacs who have symptoms consistent with gluten sensitivity, but in none of the controls tested including cow manure. Follow up surveys of those individuals with elevated stool antibodies who initiated a gluten-free diet compared with those with elevated antibodies who did not reportedly showed significantly improved quality of life and symptoms in the gluten-free group.

Knowing your DQ specific serotype pattern may be helpful for several reasons. If you have more than one copy of DQ2 or DQ8 you carry two of the major genes. For example if you are DQ2/DQ2, DQ2/DQ8, or DQ8/DQ8, a term Scott Adams of www.celiac.com has dubbed a "super celiac", you may be at much higher risk for Celiac disease and have more severe gluten sensitivity. Certainly if you are DQ2 and/or DQ8 positive you are at increased risk for Celiac disease. After a single copy of DQ2 or DQ8, it appears DQ7/DQ7 might be next highest risk. Dr. Fine has also noted some other associations with the DQ patterns with microscopic or collagenous colitis, neurologic manifestations of gluten sensitivity and dermatitis herpetiformis, which has been one of the gluten sensitive conditions noted to be at times occurring in DQ2, DQ8 negative individuals.

In the meantime, many patients are faced with whether the failure of traditional blood tests, small bowel biopsies, and the presence or absence of DQ2 and DQ8 to diagnose or exclude gluten sensitivity justifies the uncertainty and added cost of full DQ testing and stool testing. Physicians unfamiliar with this testing are increasingly being presented with the results and confused or skeptical pending published results. There also continues to be a lack of consensus in the medical community regarding the definitions of non-Celiac gluten sensitivity and what tests justify recommendations for gluten-free diet. It is clear though that gluten sensitivity by any criteria is much more common than ever thought and a hidden epidemic exists.

Each of us has two copies of HLA DQ. Because there are 9 serotypes of DQ we are all DQx/DQx where x is a
number between 1 & 9. For example, I am DQ2/DQ7. I received the DQ2 from one of my parents and the DQ7 from the other. Because we get one DQ type from each of our parents and give one
to each of our children it is easy to to see how the DQ genes pass through a family. This is important because two DQ types, DQ2 and DQ8, are estimated to be present in over 98% of all people who have Celiac disease, the most severe form of gluten sensitivity.

Again, the advantages of full DQ testing is to determine if someone has one or more copy of DQ2 or DQ8 or carry both and therefore have a higher risk for Celiac disease or more severe gluten intolerance. If you are DQ2 or DQ8 negative then your risk of Celiac disease is low, though not non-existent. If you are not DQ4/DQ4 then you do have a risk for gluten sensitivity. If you determine the full types within enough family members you can piece together a very accurate history of the origins of Celiac and gluten sensitivity within a family and make some very accurate predictions of risks to other family members.

DQ2 & DQ8, the two major types, are present in 90-99% of people who have Celiac disease. They are also present in approximately 35-45% of people in the U.S., especially those of Caucasian race of Northern European ancestry. The prevalence in the U.S. of Celiac disease is 1% and though a prevalence of 1 in 100 is very common and much higher than had been believed for years, this is only a fraction of the genetically at risk that actually are confirmed to have Celiac disease by abnormal blood tests and small intestine biopsies. However, the number of people who report a response to a gluten-free diet is much higher.

Enterolab also provides the stool testing for gliadin and tissue transglutaminase antibodies to determine if gluten sensitivity is evident. The gliadin antibody alone is
$99 or the full genetic typing, stool testing for gluten and cows milk protein antibodies, and a test for evidence of malabsorption is $349.
